In 310 bipolar hemiarthroplasties for displaced femoral neck fractures, over half had early medical complications, with older age, higher comorbidity burden, and displaced transcervical or basicervical fracture patterns independently predicting them.
In a retrospective cohort of 141 patients, mini fragment T-plate fixation of medial malleolar fractures had infection, reoperation, and nonunion rates comparable to small fragment screws, supporting it as an alternative when fragment size limits two-screw fixation.
In 25 adults aged 65 or younger with mostly high-energy displaced femoral neck fractures, InterTan nail fixation after acceptable reduction achieved 100% union and a 4% two-year reoperation rate, supporting it as a possible option.
When legal reasoning contradicts sports orthopaedic evidence.
ICCRA statement criticizes an English High Court ruling linking ankle arthroscopy during fracture fixation to a footballer's arthritis, arguing it contradicts evidence that posttraumatic arthritis follows the fracture itself and arthroscopy is standard care.
In 359 confirmed fracture-related infections, CRP was about 2.5 times higher in gram-positive than gram-negative cases, but poor discrimination (AUC 0.634) means it cannot reliably identify pathogen type; ESR did not differ.
In 450 pediatric supracondylar fractures, only 16% waited beyond 18 hours for surgery; evening ED arrival and high Gartland severity predicted delay, questioning the arbitrary 18-hour quality metric.
In a synthetic tibial fracture model, the nail's internal compression device generated and maintained more interfragmentary compression than backslapping alone, though some compression was lost after loading and results differed between nail designs.
This commentary argues the 2025 consensus review on navicular fractures in elite athletes contains errors, including a unit-conversion mistake: mean return to sport after surgery is about 19.7 weeks, not 8.2, along with eligibility-criteria discrepancies.
In 30 unstable pelvic ring injuries, minimally invasive TIFI plus INFIX fixation gave reliable reduction, 100% union, and good functional scores, with mostly minor complications; comparative studies are still needed.
A six-variable Tracheostomy Likelihood Score, driven mainly by admission AIS grade and intubation, accurately stratified tracheostomy risk after operative cervical spine trauma in a single-center internally validated cohort.
Outcomes of Surgical Interventions in Children With Fibrous Dysplasia/McCune-Albright Syndrome.
In 45 children with fibrous dysplasia/McCune-Albright syndrome (64 surgical sites), mostly proximal femur surgery for impending fractures, only 42% stayed event-free at 5 years, with higher risk in girls and younger patients.
In 780 matched uncemented THAs, dual-mobility bearings cut dislocation (0.4% vs 4.0%) but raised periprosthetic fracture risk (OR 3.5), especially in femoral neck fracture patients, so weigh trade-offs carefully.
In 4,513 Birmingham Hip Resurfacings over 25 years, 8.7% were revised; female sex and heads under 50 mm drove metal-debris failures, while males with larger heads had very low revision rates.
In 142 children with Crawford I-III congenital tibial pseudarthrosis, radiolucent lines independently predicted fracture alongside Crawford type, suggesting they can help stratify risk and guide surveillance and timing of intervention.
CT analysis of 92 humeri found the medial column accommodates at least a 5.0-mm screw in nearly all elderly patients, supporting medial column screw fixation for transcondylar fractures, though trajectories need patient-specific adjustment.
Failure Patterns Leading to Revision After Distal Humerus Fracture Fixation: A Multicenter Study.
In 411 revisions after distal humerus fracture fixation, failures were mechanical (43%), functional (41%), or infectious (16%), with open fractures strongly predicting infection and infections presenting earliest.
MRI vertebral bone quality scores depend heavily on ROI placement; calibrated posterior sampling (cPVBQ) was most reproducible and best discriminated fragility fractures (AUC 0.829), though validation in unselected populations is still needed.
Registry analysis of Australian hip fracture patients found older age, male sex, cognitive impairment, comorbidities, and aged-care residence raised mortality, while early surgery independently lowered it and delays beyond 48 hours increased risk.
Two cases of delayed structural failure, 7 and 9 years after medial femoral condyle flap distal radius reconstruction, show that radiographic union may not predict long-term durability under rigid spanning fixation.
In 29 patients, salvage reverse shoulder arthroplasty after failed proximal humerus plating improved function but recovery was moderate, with 34% complications; tuberosity deficiency and anterior deltoid dysfunction predicted worse outcomes.
Open reduction of displaced femoral neck fractures in adults under 60 gave better reduction quality than closed reduction but no fewer nonunions or osteonecrosis; age, severity, and reduction quality drove outcomes.
In matched AO/OTA 43C pilon fractures, tibiotalocalcaneal nailing and ORIF showed no detectable difference in infection, reoperation, nonunion, or amputation, supporting nailing as a limb salvage option in severe cases.
In over 524,000 cementless primary THAs, 90-day periprosthetic fracture risk rose beyond about age 71-72, which can help risk-stratify patients when choosing femoral fixation.
A commercial precontoured suprapectineal plate fit poorly (anterior overhang or gap over 5 mm) on mirrored 3D pelvic models in two-thirds of 12 Japanese acetabular fracture patients, suggesting a role for patient-specific planning.
In 349 hemiarthroplasties for femoral neck fracture, after-hours surgery was independently linked to longer operative time, more blood loss, more transfusions, and higher revision and PJI risk, while delay beyond 24 hours was not.
In China's working-age population, spinal fracture cases and disability years from road injuries rose from 1990 to 2023 even as age-standardized rates declined, with males and pedestrians bearing the highest burden.
