SUMMARY — THE REDUCTIONA case report describes successful staged four-finger reconstruction using composite flaps and iliac bone grafts in a young man, preserving all ten toes with good function and appearance at 5-year follow-up.

Multi-digit defects are difficult to reconstruct. A 25-year-old man underwent staged four-finger reconstructions using composite flaps and iliac grafts. Good functional and aesthetic outcomes were achieved with preservation of all toes. This represents a novel and effective reconstructive strategy.
